StrokePath

About StrokePath

StrokePath exists because of a gap that almost everyone who has been through a stroke recognises, and almost nobody plans for.

In hospital, recovery is surrounded by people who know what they are doing. A physiotherapist decides how someone should move. A speech therapist decides what is safe to swallow. A nurse notices the skin on a heel before it becomes a wound.

Then comes discharge, and all of that expertise walks out of the room. What replaces it is a family, a folder of instructions, and a follow up appointment weeks away.

StrokePath is built for that gap. Not to replace the rehabilitation team, but to carry what they know into the house, in a form a tired person can actually follow at six in the morning.

Our mission

To ensure no stroke survivor or caregiver feels unsupported after discharge from rehabilitation or hospital care.

What we believe

  • Practical over theoretical

Nobody in the middle of a hard week needs a lecture on neuroplasticity. They need to know how to get someone from a bed to a chair without hurting either of them.

  • Home is where recovery actually happens

Therapy sessions are a few hours a week. The other hundred and sixty are at home, and they matter just as much.

  • Evidence informed, and honest about its limits

The guidance comes from rehabilitation professionals. Where something has not yet been reviewed, the app says so rather than letting you assume it has.

  • Accessible by default, not as a feature

The audience has cognitive and motor impairment, and is often older and stressed. That shapes every decision, from the reading level to the size of the buttons.

  • Empowered by knowledge. Supported through recovery

The right information can turn uncertainty into understanding and understanding into confidence.

  • More confidence. More independence. More possibility

StrokePath is designed to empower survivors to participate in their recovery and rebuild everyday life with greater confidence.

Built by multidisciplinary rehabilitation professionals

The content library behind StrokePath was written and contributed to by rehabilitation professionals, and it is reviewed before it is published.

Recovery after a stroke is not one discipline's problem. Movement, thinking, speech, swallowing, nutrition, mood and the home itself all pull on each other, and guidance that only covers one of them leaves a family to work out the rest alone.

Where the product is today

StrokePath is not finished, and we would rather say so here than let you find out later.

The care areas, the daily plan and the education library are built and hold real clinical content. Material that carries genuine safety risk is marked inside the app with its review status, so you can see at a glance what still needs a clinician's attention.

Demonstration video is not filmed yet. Some education topics are still summaries rather than full articles. Those gaps are named, not hidden.

See what is actually in it

The programme page walks through every care area, guide and library, with the real numbers.
